Novel Concierge Health provides medical services via telehealth in states where our providers are licensed and authorized to practice.
By using our services, you acknowledge and agree that:
- Telehealth involves electronic communication and data transmission
- Telehealth may not be appropriate for all medical conditions
- Services are provided based on the laws of the state where the patient is physically located at the time of care
3. Asynchronous Telehealth Services
Some services are delivered through asynchronous telehealth, meaning clinical evaluation and prescribing decisions may occur without a real-time audio or video visit.
By using asynchronous services, you acknowledge and agree that:
- Medical decisions rely on information you submit through the EMR
- You are responsible for providing complete, accurate, and truthful information
- A provider may determine asynchronous care is not appropriate and may require a synchronous visit or decline treatment
4. Eligibility
You must be at least 18 years of age to use this website or receive services unless otherwise permitted by law and expressly agreed upon by the Practice.
5. Privacy & HIPAA
Your use of this website and services is governed by our Privacy Policy and Notice of Privacy Practices, which describe how we collect, use, and protect personal information and Protected Health Information (PHI).
Medical information is collected only through our secure EMR, not through the public website.
6. Payments & Fees
Novel Concierge Health operates on a cash-pay and/or concierge model. Fees are disclosed prior to scheduling or treatment.
By submitting payment, you agree that:
- Fees are due at the time of service unless otherwise stated
- Membership or subscription fees, if applicable, are billed according to disclosed terms
- Services are not billed to insurance unless explicitly stated
7. Off-Label Use of Medications
Some medications prescribed may be used in an off-label manner, meaning for a use, dose, route, or population not specifically approved by the U.S. Food and Drug Administration (FDA).
Off-label prescribing is a common and accepted medical practice. Medications are prescribed based on clinical judgment, available evidence, and individual patient needs. By engaging in care, you acknowledge and consent to the possibility of off-label medication use.
8. Obesity Medicine & Weight Loss Services
Obesity medicine services may include lifestyle counseling, medication management, and clinical monitoring. Individual results vary, and no specific weight loss outcome is guaranteed.
Weight loss medications may require long-term use. Availability may be affected by supply shortages, regulatory changes, or pharmacy limitations. Discontinuation of therapy may result in weight regain.
9. Hormone Replacement Therapy (HRT)
Hormone Replacement Therapy is an individualized medical treatment. Response varies between individuals, and symptom improvement is not guaranteed.
HRT may involve FDA-approved or compounded medications. Patients agree to appropriate follow-up, monitoring, laboratory testing when clinically indicated, and adherence to treatment recommendations.
10. Compounded Medications
Some medications may be compounded by licensed pharmacies. Compounded medications are customized formulations and are not FDA-approved in the same manner as commercially available drugs.
Use of compounded medications is voluntary and based on clinical judgment, availability, or patient preference.
11. Controlled Substances
Certain medications may be classified as controlled substances under federal or state law.
Prescribing of controlled substances is subject to additional legal and clinical requirements. Novel Concierge Health reserves the right to:
- Require synchronous or in-person evaluation where legally required
- Decline to prescribe controlled substances at its discretion
- Modify or discontinue therapy based on clinical judgment or regulatory requirements
Certain medications may require an in-person examination or synchronous visit in accordance with federal or state law.
12. Collaborative Practice Arrangements
Novel Concierge Health maintains collaborative practice arrangements only in states where such arrangements are required by law.
In the state of Ohio, services are provided pursuant to a valid collaborative agreement with a licensed physician, in accordance with Ohio law. In other states, services are provided by licensed clinicians practicing under the applicable scope-of-practice and telehealth laws of the state in which the patient is located at the time of care.
13. No Guarantees
Medical outcomes vary. Novel Concierge Health makes no guarantees regarding treatment outcomes, symptom improvement, or weight loss results.
14. Intellectual Property
All website content, including text, graphics, logos, and branding, is the property of Novel Concierge Health and may not be reproduced without written permission.
15. Third-Party Services & Links
We may link to third-party platforms. Novel Concierge Health is not responsible for third-party content, privacy practices, or security.
16. Limitation of Liability
To the fullest extent permitted by law, Novel Concierge Health shall not be liable for indirect, incidental, consequential, or punitive damages arising from use of this website or services.
